8/15/2023 EnchantaVaughn Approved Behavior: Ben-Nita presented with a normal spirit and affect. Ben-Nita was in a pleasant mood today and presented good behaviors, as evidence of her smiling and being polite toward her family. Ben-Nita displayed calm and peaceful expressions, as she waved hello while presenting a respectful tone toward mental health counselor. Behavior: Ben-Nita was sociable and respectful to her family during the family activity. Ben-Nita was excited to express her feelings and share her favorite thing she loves about having counseling as a way to help her understand how it keeps her grounded and able to deal with things internally that she didnât know how to do before. Intervention: QMHP-C went to the home to meet with Ben-Nita to discuss her recent progressions. QMHP-C engaged in conversation with Ben-Nita about factors that contribute to ADHD. QMHP-C utilize positive self-talk strategies to help Ben-Nita think positively at all times and to talk about her actions and illness. QMHP-C utilized active listening skills while Ben-Nita was verbalizing information she learned. QMHP-C provided simple praises to Ben-Nita for her commitment to her progress. The QMHP-C demonstrated and provided positive and empathetic acceptance. QMHP-C reviewed the positive things that Ben-Nita is doing and encouraged her to continue in making positive decisions in the home. Intervention: QMHP-C gave Ben-Nita the opportunity to express and explore her perceptions of how her symptoms of ADHD can affect her. QMHP-C continued to monitor Ben-Nitaâs relationship with mother and compliance to duties and expectations for the Ben-Nita in the home and in the community. QMHP-C encouraged Ben-Nita to use her strategies and skills with peers, family, and relatives to support her with managing symptoms associated with ADHD. QMHP-C provided a list of experiences that is common with her mental illness. QMHP-C continued to elaborate on ways Ben-Nita can cope and manage stressors. Response: Ben-Nita and mom were observed interacting well with one another before giving QMHP-C their attention. Both Ben-Nita and guardian provided positive feedback, actively participated, and engaged with QMHP-C. Ben-Nita was receptive and open to the interventions, as she was focused on the information and activities that were being processed without any difficulties. Response: Ben-Nita and guardian were acceptive of QMHP-C interventions. Ben-Nita appears to be calm and relaxed after the interventions were implemented. Ben-Nita shared some of the things she does at home to help her relax. Ben-Nita states that going to dance practice, getting her nails done, getting her hair done all make her feel relaxed. Ben-Nita presents moderate self-esteem throughout the session, as evidenced by utilizing creative strategies to stay motivated and on task. Ben-Nita communicated that he felt safe and comfortable talking to the in-home counselor. Ben-Nita acknowledges expectations and reviews goals and objectives with counselor and parent.
